Most traffic department services start with a prescribed NaTIS form. The forms are national, so a DL1 is the same form in Durban as in Polokwane, but centres differ on whether you complete it beforehand, collect it at the counter or have it done on screen. This library lists the NaTIS forms by their official titles, explains which one you need, and shows where to download them free.

What this form is used for
NaTIS forms fall into three groups. The official titles below are as listed on natis.gov.za.
Driving licences and permits
- LL1, “Application for learner’s licence”. The LL1 application form, also searched as the learners licence application form LL1, is the most requested form here. See the LL1 form.
- DL1, “Application for driving licence”, also used for licence card renewal. See the DL1 form.
- PD1, “Application for professional driving permit”. See the PD1 form.
- MC, “Medical certificate”, for PrDP applicants and some learner applicants. See the MC form.
- Optometrist form: an eye test result from a registered optometrist. See the optometrist form.
Vehicles
- ALV (sometimes searched as AVL forms), “Application for licencing of motor vehicle”, used when no renewal notice arrived. See the ALV form.
- NCO, “Notification of change of ownership / sale of motor vehicle”, completed by the seller. See the NCO form.
- RLV, “Application for registration and licencing of motor vehicle”, completed by the buyer. See the RLV form.
- DRC, “Application for duplicate registration/deregistration certificate”.
- ADV, “Application for deregistration of vehicle”, and ACR, “Application for certification of roadworthiness”.
- TSP1, “Application for temporary / special permit”, also written TSP 1 form. See the TSP1 form.
- MVL1 and MVL2: MVL2 is the licence renewal notice you receive, and some counters also ask for an MVL1, the application for motor vehicle licence (MVL1) form. See MVL1 and MVL2.
Personal details and declarations
- NCP, “Notification of change of address or particulars of person or organisation”. See the NCP form.
- DCT, “Declaration in respect of lost documents”. See the DCT form.
- ANR, “Application and notice in respect of traffic register number”, for people without a South African ID number. See ANR.

Where to get the official form
NaTIS forms, also called eNaTIS forms, are downloaded free as PDFs from the NaTIS road traffic forms page on natis.gov.za, not from NaTIS Online. An LL1 form download, an NCP form download and a general NaTIS forms download all come from that one page.
The same forms are free at the licensing centre or registering authority counter, and many provincial and municipal websites republish them. Never pay a website for a blank government form; copies from unofficial sites can be outdated versions that a counter will reject.
How to complete it
- Use black ink and capital letters, and write within the blocks.
- Use your ID number and names exactly as they appear on your ID document.
- Do not sign in advance where a form must be signed in front of an official.
- Leave sections marked for official use empty.
- If you make a mistake, start a clean form rather than crossing out.
Documents to submit with it
